File a claim

You can submit a claim if, as a result of your Phoenix pay issues, you have:

  • incurred financial costs or lost investment income
  • taken leave due to health issues, or
  • suffered severe damages and personal hardship

File your claim manually

Download the PDF version of the form and file your claim manually if you:

  • do not remember your personal record identifier (PRI)
  • do not have an email address
  • are filing a claim on behalf of someone else
  • are using an international mailing address

Track a claim

To track the status of your claim, you will need your personal record identifier (PRI), date of birth, and your claim number.

Add to an existing claim

You can add another type of claim to your existing claim up until the claim has been closed. To add to an existing claim, you will need your personal record identifier (PRI), date of birth, and your claim number. Note: Any information previously submitted will be unavailable for editing.

Respond to a request for more information

If you receive a letter via regular post mail from the Government of Canada requesting more information to better process your claim, you can respond to that request here. To respond to a request for additional information you will need your personal record identifier (PRI), date of birth, and your claim number.
